I am working with a large organisation who are at the start of a large project that aims to implement an integrated social care and education case management solution that replaces numerous disparate systems ensuring best-practice business processes, migrated data from multiple systems, trained staff from a number of agencies on optimum use of the solution and integration with key systems using open standards.
The aim of the solution is to create a 'single version of the truth', creating one record for people who need support so they do not get lost in transition between services or in different phases of their life. It will support citizens and clients, providing the ability to adopt a 'tell us once' approach across all required services and agencies. The solution will also support accurate evidence-based commissioning across all people-based services.
